This is a compromise that often will address the needs of both vendor and customer. General Hospital can have remote access without incurring additional charges. Presumably, if the long term care facility's usage gets so high that the hospital wishes to install a separate copy at that site, then it will also be worthwhile to pay some fee for that copy. At the same time, BHS gives the customer the flexibility that it requests but still reserves the ability to charge later on when the demand requires a separate copy of the software. As a practical matter, the hospital probably would not be willing to pay an additional license fee for the small facility before that point anyway.
